Babies and Blankets

Home visits for families who have children younger than five years who are looking for extra parenting support.

Children With Special Health Needs Home Visits

Follow-up nurse visits for families with children having special health needs. This may include parenting information, health information and connection to other community resources.

Early Head Start Home Visits

Weekly home visits focusing on child development and parenting support.

ECFE/Family School Home visits

Free home visits for families with children birth – 5. An ECFE teacher will provide parenting support and answer any questions you may have regarding your child’s development.

Family Home Health Aide Program

Family Home Health Aides provide one-to-one support to strengthen the family unit by working closely with family members on areas such as behavior management, building self-esteem, homemaking skills, and parent-child activities.

Hand in Hand Home Visits

Free home visits for current Hand in Hand students by a Hand in Hand teacher with a focus on literacy skills. Free book with every visit.

HealthFinders Home Visits

HealthFinders Collaborative, is a community health center which provides a comprehensive primary medical and dental care, wellness, and patient assistance including insurance enrollment, to those with limited healthcare alternatives. HFC services include home visits by community health workers for those that would benefit from such support.

Scholarships/Financial Assistance: Many services are free, otherwise HFC has a sliding fee discount based on income and family size; no patient is turned away regardless of their ability to pay.

Public Health Newborn/Postpartum Home Visits

Public Health Nurses provide support and education to new parents after hospital discharge.
